Step-By-Step Process of a K1 Fiance/Fiancee
Medical Exam
One of the most important requirements for a K1 Fiance/Fiancee
Visa Petition is a medical examination done by the St. Luke's Extension Clinic
located at Ermita Manila, the only accredited medical institution approved by
the US Embassy in the Philippines.
All K1 Fiance/Fiancee Visa Applicants, regardless of age,
need to pay for the medical examination fee and complete a medical examination
at St. Luke's Medical Center Extension Clinic (SLMCEC) before they will be
interviewed by an American Consul at the U.S. Embassy in Manila.
You can reach them by dialing their telephone numbers at
521-0020 and 521-8647. All applicants are advised to have their medical
examinations done before their interview appointment at the embassy.
Take the following items with you:

Your passport,

Four passport photos – 2 for the K1 package, 2 for the St. Lukes Patient Data
Sheet (PDS),

Your K-1 Visa application materials,

Around 12,,000 pesos to pay for the medical exam and immunization (you should
keep the receipt in case she has to go back the next day),

Your Notice of Approval (NOA) #2 (original or faxed copy), or a letter from the
Embassy indicating you have an appointment for an interview.
SLMCEC will require applicants to present their passport
(valid for at least 6 months) and one photocopy of it, one 2x2 Picture in plain
white background and another two 2x2 picture upon the release of the result and
an appointment letter sent by the National Visa Center with one photocopy of it.
It is advisable that you bring a few additional pictures,
their inexpensive and it is better to have more.
Your First Day At St. Luke's Medical Center Extension Clinic (SLMCEC)
St. Luke's Medical Center Extension Clinic (SLMCEC) is
located at 1177 J. Bocobo Street, Ermita, Manila. Applicants are served on a
"first come first served basis". When you'll arrive at SLMCEC you'll notice that
there will be lots of people already waiting as the lines do start forming as
early as 3am so better come early because if you arrive as late as 5am you may
not be aloud in for that day and will have to return the following day.
The guard will ask for your appointment letter and passport
so you need to prepare these documents for easy access when you'll arrive in the
clinic. After the guard could check these documents, he will give you a queue
number and forms that you'll need to fill up.
The clinic information attached in the Embassy packet could
instruct applicants that St. Luke's Extension Clinic opens at 7:00 AM, but don't
be surprised if they'll open earlier than that and start to work right away
because they open early when they see people outside the clinic waiting.
The next thing that you'll do is to fill-up the Patient Data
Sheets Forms (PDS) and after, you need to wait for awhile. If your number will
be called by another guard then you'll be instructed to proceed to another room
to have your picture taken. Each visa applicant should be taken a photo shot
inside the clinic.
After your picture will be taken, you need to form a line among other visa
applicants. Always be ready with your passport. You will present it with every
transaction you'll make at St. Luke's. The receptionist will check your forms
and the required documents.
She will also ask you some important questions (i.e. if you
had allergies or if someone in your family has allergies, when was the last time
you had an X-ray, if it was your first time having a medical exam at St. Luke's,
and so on). After the preliminary interview, you'll be instructed to proceed to
the 5th floor in order to pay the fees at the Cashier's Section.
The payment should be in Philippine currency, always check
for the current fees. When you finished paying the necessary fees, you need to
submit your papers to the "USA Counter" and then proceed to the room where your
physical exam will take place. Your height and weight will be taken then there
will be an eye test, blood pressure taken, and the last will be the physical
exam where you'll be one on one with the clinic's doctor.
Usually, they assigned female doctors for their k1 female
applicants. You'll be checked and interviewed about your medical history and
she'll also check your breasts for lumps and genital area very quickly for
venereal disease. It won't take long so try to relax during the whole process.
You'll be given a cup for a urine sample to be collected
after. This could be given for a pregnancy test. After this you'll be directed
to another room for a blood sample collection. This could be given for HIV/AIDS
testing. After the urine and blood sample collection, your name will be called
and you need to get your pertinent papers.
Next is the submission of these papers at the counter for the
X-ray to be taken. Your name will be called based on your waiting number and
you'll proceed to the X-ray room. You need to wear clothes that can easily be
taken off. You'll remove everything except for underwear and change into a
hospital gown. During the X-ray, you'll be given a protective shielding for your
abdomen.
After your X-ray you need to bring the papers to the 2nd
floor where a vaccination interview will be conducted. The doctor will ask if
you are allergic to anything before giving you the
option to either have the vaccination shots at St. Luke's or in the US. Most K1
visa applicants will get them at St. Luke's since they need to pay them whether
they'll get it or not.
Many applicants will finish all the tests in the morning
especially if they come early too. After all the tests had been done, you'll
need to go to the check out counter where you'll be informed when to come back.
If you have previous vaccinations, you need to bring your records and it should
be signed by your doctor with his/her license number attached and it doesn't
need to be notarized.
Usually, you'll be scheduled the following day to pick-up the results of the
medical exams and receive your vaccination shots too. Remember that you can't
submit your visa application in the US Embassy without the results. Make sure
that you'll receive a receipt so that you can show it to the guard when you'll
return the next day.
Your Second Day At St. Luke's Medical Center Extension
Clinic (SLMCEC)
You need to show your receipt and passport to the guard at the ground floor and
it will be checked on the computer. You'll be instructed to sit and wait for
your name to be called for your immunization shots of the day (mumps, measles,
rubella, etc). The waiting time could take a bit longer so be sure to eat your
breakfast so you won't get hungry.
When your name will be called, you need to submit your
passport and medical receipt to the doctor in-charge of the immunization and had
to sit down and wait again. Your name will be called and you'll be instructed to
go into the immunization room to receive your shots.
The doctor will inform you that you should not get pregnant within the next four
months because the vaccinations may cause abnormalities in the unborn child.
After that, your name will be called at Counter A. Submit your passport and two
2x2 photo with white background and if possible, similar to the one you attached
in your Patient Data Sheets Forms (PDS) with your full name at the back of each
photo.
You need to wait again for your name to be called at Counter
E. When your name will be called, you'll be asked some questions probably for
verification purposes because they will check your answers based on all the
pertinent papers that you had submitted to them. After that, they will give your
X-ray film and vaccination sheet. SLMCEC will be the one who'll forward your
medical results directly to the US Embassy, together with your passport in 2-3
working days.
